A Discharge Advice Slip will need to be taken to the Discharge/Cashiers desk on Level 1 and returned to the nurse in charge of your ward just prior to leaving; or a friend or relative can do this for you when they come to pick you up. All accounts will need to be settled before you leave the hospital and it may also be necessary for you to sign forms relating to your health insurance. If you are a private patient please make sure you are aware of what your health insurance does and does not cover. If you are unsure please contact the Patient Liaison Officer on ext 4807.

Nursing/midwifery staff will discuss your discharge with you a few days before you leaveso that appropriate arrangements can be made.

Discharge time is 10:00am.

Discharge Against Advice

Except in certain cases (e.g. infectious disease), every patient has the right to leave hospital when he/she chooses. This may be a serious step when taken against the advice of your doctor and requires great caution. If you choose to discharge yourself you will be asked to sign a disclaimer, because the responsibility would rest entirely with you. If you decide to discharge yourself against the advice of your doctor and your condition does not improve, or causes you concern, seek further advice. If your condition worsens dramatically contact the hospital’s Emergency Department on 03 8458 4000 or go to the nearest emergency department.
