  • Referrals from Health Professionals

    Health professionals and community providers send referral forms directly to Mercy Health O’Connell Family Centre.

    The health professional is responsible for advising the family they have made a referral to Mercy Health O’Connell Family Centre.

    There are criteria for admission to our services and some families seeking admission may not meet these criteria. There is also a waiting period for all our services. 

    Each family has an initial intake assessment by the Triage MCHN and is then offered the most appropriate program to meet their needs based on the admission criteria. 


    Referral Procedure

    Once the referral has been made, our triage nurse will liaise with the health professional if we are unable to contact the family, or require further information regarding the family. 

    The health professional will also be notified if the family does not meet the criteria for admission.

    A pre-admission consultation may be booked if required, either onsite or over the phone, depending on the location of the family and this will include a tour of the facility.                              

    Our clinical team consists of maternal and child health nurses, midwives, mothercraft nurses, early childhood workers, a psychiatrist and a psychologist. 

    Our team may contact the referring professional during the family’s stay, providing progress updates as required. Case meetings can be held with invited participants.                             

    A discharge letter will then be sent to the referring health professional and other professional as required.  

    A discharge phone call is provided to all families attending Residential programs.
